Many business analysis project fail because too many people are assigned to the project.  Large projects are difficult to manage.  As each additional person is added to a project, the communication time and effort needed increases disproportionately:

Business analysis projects should be kept small. The people assigned to business analysis projects, however, must be empowered to represents their respective areas and to make decisions. 

Business analysis projects should not be combined with system development projects.  It is important that business requirements be well defined before costly decisions are made to purchase packages, create custom-built applications or enhance existing application systems.  The outcome of some business analysis projects often will be to make organizational, policy or procedural changes - not changes to computer systems.
